This form is required for all Beach Brew employees. These documents are very important for payroll. You must complete this form before you are scheduled, as you will not be eligible for pay until you complete the form.
To put it simply, you will have to upload between 1-3 different types of documentation depending on what you can provide. We suggest just uploading a List A type document. If you do not have a List A document you can provide, you will have to provide BOTH List B AND List C.
